Leukemia & Lymphoma Society travel funds open for Rutherford County

The Leukemia & Lymphoma Society recently announced the creation of the Susan Lang Pay It Forward Patient Travel Assistance Program for blood cancer patients who reside in Rutherford County.

 

Funding, which opened on August 30, for the program comes from the Christy Houston Foundation.

 

ELIGIBILITY:

 

  • Patients must be a citizen or a permanent resident of the U.S. and reside in Rutherford County
  • Patients must have a blood cancer diagnosis confirmed by a doctor 
  • Patients may be insured with public or private insurance or, uninsured
  • Patients must have a household income that is at or below 500% of the U.S. Federal Poverty Guidelinesas adjusted by the Cost of Living Index (COLI)

 

Approved patients are awarded a grant of $500, which is distributed via the Susan Lang Pay-it-Forward Patient Travel Assistance credit card. The card can be used to pay for approved travel expenses, including ground transportation, tolls, gas, parking, car rental and repair, air transportation and baggage fees, lodging, and ambulance services. Expenses not covered by the program include food, drinks and alcoholic beverages, tobacco products, auto dealers, travel agencies, and international travel. 

 

Applicants can apply for an award every six months, as long as funding is available.

 

If a patient has an active travel award through the general travel fund, they are not eligible for this program as patients can only have one active travel award every six months.
