POST-EXPOSURE EVALUATION AND FOLLOW-UP

Should an exposure incident occur, contact Dr. Andrea Anthony, Assistant Superintendent of Human Resources and Student Services at (615) 893-5812.

Following initial first aid (cleaning the wound, flushing eyes or other mucous membranes, etc.), the employee should:

• Notify employee’s immediate supervisor, the school principal, and Assistant Superintendent for Human Resources and Student Services Dr. Andrea Anthony at Central Office.
• Complete an “On-The-Job Injury” Employee Injury Statement.
• Refer to the ECP document for further instructions.
