Asthma 

If your child has been diagnosed with Asthma or will be taking any medications for asthma at school, please have these forms filled out as soon as possible. Most require a physician's signature.  If your child has medications, they must be signed in at school by a parent/guardian.

Allergy or Anaphylaxis 

If your child has been diagnosed with a Severe Allergy or Anaphylaxis, or will be taking any medications at school, please have these forms filled out as soon as possible. Most require a physician's signature. If your child has medications, they must be signed in at school by a parent/guardian.

Cardiac conditions  

Please fill out this form to inform the school of any heart conditions your child may have.  Have your physician sign where applicable. If your child has medications, they must be signed in at school by a parent/guardian.

Diabetes 

If your child has been diagnosed with Diabetes (Type I or Type II), please come to the nurse's office on the first day of school to discuss individual orders.  If your child is under the care of the Vanderbilt Pediatric Endocrinology Department, they should provide orders for school use.  If your child is being treated by anyone other than those at Vanderbilt, please use these orders (below). These can be taken to your doctor to fill out.  Once completed, bring them to the nurse's office and we will complete all other paperwork. If your child has medications, they must be signed in at school by a parent/guardian.

Seizures 

If your child has been diagnosed with a Seizure Disorder, please come to the Nurse's Office to discuss doctor's orders and a Safety Plan. Below are a few forms you can have filled out before school starts. If your child has medications, they must be signed in at the school by a parent/guardian.

Sickle Cell Anemia

If your child has sickle cell disease, below is the action plan. Please fill it out and have the physician fill out their section and return it to the school nurse.

Any other health related concerns should be addressed on the purple health forms that will are provided every year at registration. If you have any additional health-related concerns regarding your child, please contact the school nurse.
